Records Request Process

To request medical records for a client of SAGE Counseling, we provide the following procedure:

1. A client completes a Release of Information (ROI) authorizing the release of their medical records (Autoridad de Revelar Información)
2. A request for medical records is sent to [email protected].
3. SAGE Counseling will process records requests within 5 business days of receiving a records request and valid ROI.

Medical records approved for release are provided at no cost.
