Discounted Fee Application
For clients who would like to apply for discounted rates, complete a Financial Assessment Application for determination of sliding fee eligibility.
Billing Statement
A Financial Eligibility Specialist will send a copy of your billing statement to the email provided within a business day of the request. If there are any additional questions about your summary, please respond to the email you will receive or give us a call directly at 602-761-4930.
