Transition Support
Re-Entry/Transition Services
Through a contract with the Arizona Department of Corrections, SAGE Counseling provides community-based transitional services for released inmates in Arizona.
SAGE provides an evidence-based and gender specific continuum of services that includes:
• Assessments
• Information Sessions
• Case Management
• Delivery of Treatment and Psycho-educational Programming.
These services are provided state-wide. Live groups and classes are held in Maricopa, Pima, Pinal, Gila, Yavapai, Coconino, Mohave and Yuma Counties.
We serve a wide range of offenders and provide assistance with employment, educational/vocational training, obtaining food boxes, mental health/medical services, medication needs, public assistance programs, faith based services, twelve step groups, housing needs, and literacy tutoring.
Substance Abuse Treatment Services in Coconino, Mohave and Yavapai Counties
Through another contract with ADC, SAGE provides an effective evidence based community outpatient substance abuse treatment program to offenders, aimed at reducing offenders’ use of addictive substances upon their release from prison and minimizing/reducing or preventing relapse, revocation and recidivism.
Services are for offenders who reside in Coconino, Mohave and Yavapai Counties, and consist of:
• Assessments
• Group Counseling
• Individual Counseling
• Contract also allows for education classes, and family and relapse prevention groups
